One of the most important steps in the Insurance process is to work with your Doctor and Therapist.

Steps to Work with your Healthcare Professional

Start here! Let us guide you through the required documents that you need to get from a Doctor/ Therapist for Getting zPod funded through Insurance/Medicaid!

  1. Download the documents and take a print of the REQUIRED and RECOMMENDED documents/ prescription to take to your doctor
  2. Setup an appointment with the Doctor to Get Prescription FILLED & Duly Signed
  3. You have Necessary documents for Insurance!
  4. Collaborate with DME (medical provider) to Submit the Documents + Prescription
  5. DME will submit the required documents ( Prescription, LMN + Others) to your Insurance Provider
  6. After Insurance approval zPod will be delivered to you!

01 Required Documents

  1. Patient Intake Form 

Physical exam and chart notes supporting the medical need for the zPod Safety Bed.

  1. Letter of Medical Necessity

A Letter of Medical Necessity (LMN) is a written letter of explanation by your child's treating

physician, specialist or therapist describing the medical treatment for the patient's care.

These letters explain the patient’s specific needs and why a zPod Safety Bed is medically

necessary. Please note, If the LMN is written by the patient’s Occupational Therapist or

Physical Therapist, the prescribing doctor will need to sign as well. See what to include in the

LMN below:

LMN’S for zPod Safety Bed should include:

● zPod Safety Bed prescribed by the treating physician

● Medical explanation demonstrating the need for the zPod Safety Bed

● Medical rationale and diagnosis linking the requested zPod Safety Bed to the

patient's condition

● Evidence of a physical exam performed and office visit notes from the past year

  1. Prescription (Rx)

A written Rx from Primary Care Doctor/Pediatrician specifically stating a zPod Safety Bed. Please see attached written prescription template to use for a zPod Safety Bed.

  1. Written Order

A written communication from a treating practitioner that evaluates and documents the need and safety concerns for a patient to be provided an item such as a zPod Safety Bed.

  1. Patient Documentation

Physical exam and chart notes supporting the medical need for the zPod Safety Bed.

  1. Copy of Insurance Card

If your child has Primary Insurance coverage, we will need a copy of the front and back of the card. If your child has Secondary Insurance Coverage also, we will need a copy of the front and back of this card as well.

02 Additional Recommended Documents

Supporting evidence of medical need for a zPod Safety Bed.

Parental/Caregiver Personal Experience Letter. 

May also include pictures, videos as to the medical need of a zPod Safety Bed.

Share details about the challenges being faced, such as sleep issues or safety concerns. Highlight how zPods can provide a safe and sensory-friendly sleep solution.

03 Do you provide templates for the required documents?

Yes, we can provide templates or examples for the Letter of Medical Necessity and other required and recommended documents to make the process easier.

04 Does the doctor need to fill out any specific forms?

In most cases, the doctor will need to complete insurance-specific forms to confirm the medical necessity of zPods. Check with your insurance provider for any additional requirements.

05 What should I do if my doctor is unfamiliar with zPods?

Provide them with informational materials about zPods, including its benefits and how it supports individuals with specific needs. We can supply these resources if needed.

06 How long does it take for the doctor to complete the documents?

The timeline depends on the doctor’s schedule, but we recommend following up within a week if the documents haven’t been completed.

07 Can you assist in communicating with my doctor or therapist?

Yes, we’re happy to provide guidance or additional materials to support the conversation between you and your healthcare provider. Please feel free to reach out to us.

08 What happens if my doctor or therapist declines to sign the documents?

If your doctor or therapist declines, ask for their reasoning and consider seeking a second opinion from another healthcare provider who understands your needs.
